Each dental assistant must complete their own enrollment form. We'll also email this form to them, but you can copy the link below and share it directly with your staff.

Complete Your Doctor Verification Form
As the supervising dentist, complete and sign the doctor form confirming your dental assistant's qualifications and your commitment to supervision.
âšī¸ Note: Only fill out ONE doctor form regardless of how many staff members you're enrolling. You do not need to complete a separate form for each student.
